License domain entry page is broken

When you attempt to download freshly created (that doesn't have domain association) In-Portal license from https://www.in-portal.com/my_account/licenses.html page, then you'll need to enter a domain for that license on https://www.in-portal.com/my_account/license_domain.html page.

There are several issues with that page:

1. wrong styles used
2. wrong navigation bar used (I suspect, that we might need to remove it, because I haven't seen navigation bar on website at all)
3. when you enter domain, then instead of confirmation page (or being redirected to license download page) you'll see 404 page
